At the very least, this would seem to show extreme carelessness (deliberate negligence?) on the part of prison officials who were supposed to be actively trying to prevent a second suicide attempt.

Epstein's Death Under InvestigationAttorney General William Barr said he was "appalled" that Epstein's died of apparent suicide while in federal custody at the MCC, which is part of the U.S. Bureau of Prisons facility under the Department of Justice. “Mr. Epstein’s death raises serious questions that must be answered," Barr said in a statement. "In addition to the FBI’s investigation, I have consulted with the Inspector General who is opening an investigation into the circumstances of Mr. Epstein’s death." U.S. Sen. Ben Sasse, a Republican from Nebraska, in turn, took Barr to task over the affair with a stinging letter to the attorney general demanding that "heads must roll." "Every single person in the Justice Department — from your Main Justice headquarters staff all the way to the night-shift jailer — knew that this man was a suicide risk, and that his dark secrets couldn’t be allowed to die with him," wrote Sasse, who is chairman of the Senate Judiciary Committee. www.yahoo.com/news/jeffrey-epstein-found-dead-jail-132552127.html

Here's a direct quote from the article cited below the quote: The Justice Department would not confirm to HuffPost whether Epstein had been on suicide watch, but both The Associated Press and The New York Times, citing unnamed sources, report that he had been taken off suicide watch at some point before his death. www.huffpost.com/entry/jeffrey-epstein-dead_n_5d4ebeefe4b0fd2733f100c8I think they really need to figure out "who" took Epstein off of suicide watch and "why", if the why is still relevant once the who is known. edited to add: I went back to read the article Mel cited above and it tells this: Now I'm wondering how many people had access to Epstein prior to his suicide?
